I have only one sheet of the form... because my director neglected to send the part with the address on it

I REALLY do not want to talk to this woman again, so if someone else could help me that would be great.

Even just a push in the right direction to finding it would be nice.

Just call the return department at MK Canada and get the address from them. smiley

And while you're talking to them, verify the amount you can send back.  You may want to get them to send you a new form since it sounds like the one you have isn't complete. Wouldn't want to jeopardize your return eligibility.
