Here you go (forgive any typos as I'm not copying and pasting but rather typing what is on my photocopy of my form I used in August of this year).

"I represent and warrant that the eligible products returned to the Company for repurchase with this form are products that I have purchased directly from the Company as an Independent Beauty Consultant.  I authorize the Company to pay the repurchase amount by check made payable to me and such other person, if any, that I have listed below as an alternate payee, and mail to mail the check to me at the address listed below.  I UNDERSTAND AND AGREE THAT ANY PRODUCT OR ITEMS I RETURN TO THE COMPANY THAT ARE NOT ELIGIBLE FOR REPURCHASE IN ACCORDANCE WITH MY INDEPENDENT BEAUTY CONSULTANT AGREEMENT WILL BE DEEMED ABANDONED BY ME AND SUBJECT TO THE COMPANY'S DISPOSTION.  I represent that all of the information on this form is true and correct.  I also agree to indemnify and hold the Company harmless from any claims or damages arising out of or relating to the payment of any repurchase amount."

That part (what I typed out) was the last section before you sign.  There is another line, rather buried if you ask me, that says "Consultants who return merchandise to Company are not eligible to rejoin Mary Kay as a Mary Kay Beauty Consultant."

The form also specifies that product must be sent back via ground transportation (post office, UPS, etc).
